Volunteer Background Check and Authorization Form
The safety of the girls is the most important consideration in the appointment of a volunteer for the Girl Scouts of Black DIamond Council. To help safeguard the girls in our care, the Council has joined other youth-serving agencies by conducting background checks on volunteers. Information obtained will be handled by Council staff in a coordinated manner.

I understand I have the option of entering my personal information directly to the Council's reporting agency via a secure website or have the Council run the check for me.
I hereby give my permission for the Council and its agents to obtain information relating to my criminal history. The background check history record, as obtained from the reporting agencies, may include arrest and conviction data as well as plea bargains and deferred adjudications and may include driving records. I understand that this record will be used, in part, to determine my eligibility for a volunteer position with the Council. I also understand that as long as I remain a volunteer with the Girl Scouts of Black Diamond, background checks may be repeated at any time.
I hereby release and discharge Girl Scouts of Black Diamond and each of its officers, directors, employees, and agents from and against any and all causes of action, liabilities, and claims resulting from the investigation of my background check and further authorize any third parties, who may be the custodians of/in possession of the requested information, to disclose such information in connection with this authorization.
I understand that if the Council chooses not to extend an offer of volunteer position to me based upon the information obtained pursuant to this authorization, I will be notified by Council of pertinent non-confidential information along with a summary of my right to contest such information. I understand that, although furnishing my Social Security number is not optional, it shall be used for NO other purpose than to make the process for conducting a background search more accurate, and that the Social Security number shall not be revealed to a third party except for the express purpose of conducting the background check.
