Program Equipment Rental Request

This form should be completed by the leader or adult responsible for renting equipment.  To request program kits, please complete the following information.  Most equipment is available quickly, however, please allow up to 3 weeks for equiment to be available to rent.

Program equipment is available on a first-come first-served basis only.  It is to be used for GIrl Scout activities only.  The payment and deposit fees are required at the time equipment is received.  The rental period starts the date you requested to pick it up, not from the date of actual pick-up.  This request does not guarantee that the equipment will be available on the dates that I have requested.
