Service Unit/Cluster Event Report
For Service Unit/Cluster Events more than $15 per Girl

After the event, the Event Director should complete this form and submit no later than two weeks after the event.

  11. Was First Aid Administered at the event? If yes, please send a copy of the first aid report to GSBDC immediately.

  12. Were evaluations received from participants and staff? Please summarive the evaluations.
  13. Was girl/adult planning used for the event? Did girls provide input into the activities of the event?
  14. Overall, what was your opinion of the event conducted?
  15. Income received from girl registration fees
  16. Income received from adult registration fees
  17. Income received from program assistant registration fees
  18. Income received from other registration fees
  19. Additional Income from Event Please list the amount of any other income and the source
  20. Total Income from Event
  21. Expenses for Insurance If needed
  22. Expenses for Site Rental for Facility If a site rental was charged
  23. Expenses for Program Consultants/Activity Specialists If provided
  24. Expenses for Meals/Snacks If provided
  25. Expenses for Postage If provided
  26. Expected Expenses for Admission Fees (i.e. swimming, movies, bowling, etc., if provided)
  27. Expenses for Patches If provided
  28. Expenses for T-Shirts If provided
  29. Expenses for Activity Supplies
  30. Other Expenses Please list any additional expenses and the amount
  31. Total Expenses for Event Add all of the expenses to calculate the total expenses for the event.
