Service Unit/Cluster Event Report
For Service Unit/Cluster Events less than $15 per Girl

After the event, the Event Director should complete this form and submit no later than two weeks after the event.

  5. Date of Event
  6. Event Fee for Girls
  7. Event Fee for Adults
  8. Number of Girls Attending Event
  9. Number of Adults Attending Event
  10. Number of Non-Registered Youth Attending Event
  11. Was First Aid Administered at the event? If yes, please send a copy of the first aid report to GSBDC immediately.

  12. Were evaluations received from participants and staff? Please summarive the evaluations.
  13. Was girl/adult planning used for the event? Did girls provide input into the activities of the event?
  14. Overall, what was your opinion of the event conducted?
