Notification of Service Unit/Cluster Event
For Service Unit/Cluster Events more than $15 per Girl

Before the event, the Event Director should complete this form and submit at least four weeks before the event.

  9. Event Fee for Girls If less than $15 per girl, please submit online using the other form.
  10. Event Fee for Adults
  11. Mail Event Registrations to: Please provide the name and address of person handling registrations.
  12. Make Checks Payable to:
  13. Estimated # of Girls Attending
  14. Estimated # of Adults Attending
  15. Estimated # of Non-Girl Scout Youth Attending
  16. Has extra insurance been purchased, if needed? Extra insurance is needed if the event will have non-Girl Scout participants (girls or adults) or is longer than 2 nights

  37. Total Expected Income for Event Please list the combined total expenses expected for your event.
